Teach youth a curriculum of skills, including social, self-help, independent living and academic skills. Qualifications: Knowledge and Experience:
- Must be twenty-one (21) years of age;
- Bachelor’s Degree from an accredited university or college; or,
- Associate Degree in related field and one (1) year experience working with client population; or,
- High School Diploma and three (3) year experience working with client population.

Major Job Responsibilities: Accept and evaluate referrals for outpatient services, conferring with referral sources as needed to determine the client’s treatment needs. Provide comprehensive clinical assessments in response to initial referrals and maintains outpatient therapy caseload. Provide individual, group and family therapy to the clients receiving CABHA Outpatient Core Services in accordance with their needs, psychological abilities, and written treatment plan. Assist in the formulation of each client’s overall treatment plan, discharge criteria, and continued service recommendations via consultation with the CABHA Medical Director and Clinical Director. Develop the written treatment plan for each client. Document any and all clinical services, communication, and other client-related contacts in the Medical Record within 24 hours of the service delivery. Documentation shall be in accordance with the DHHS Records Management and Documentation Manual, with LME/MCO requirements, with agency Policies and Procedures, and with the agency’s electronic records system. Conduct utilization review activities necessary to obtain authorizations for client services by third party vendors/payers. Oversee FACT’s First Responder services to meet the needs of any non-residential clients in crisis or emergency situations in accordance with each client’s Crisis Plan. Is available, on a rotating basis, to provide required On-Call services for FACT’s clients. Collaborate and consult with the agency’s Medical Director and Clinical Director in service delivery areas and on-going treatment planning for Outpatient clients. Maintain appropriate on-going contacts with parents/guardians/case managers/LME staff or other relevant members of the System of Care as related to each minor client’s treatment needs. Fulfill any and all supervision requirements established by the relevant Board in the State of North Carolina to either obtain or maintain required clinical licensure/certification. Maintain necessary continuing education requirements in accordance with Core Services requirements and/or relevant NC State Licensure/Certification. Attend both internal and external treatment team meetings, multidisciplinary meetings, hearings, school conferences or other consultations as needed. Assist in developing and maintaining productive community-based resources to support each client’s treatment goals as well as the objectives of the program and agency. General Job Summary: The Qualified Professional in the Day Treatment Program setting, provides direct professional treatment services to the children and adolescents served by FACT. Individuals served in the DTP range in age from 8 to 18 years and exhibit significant mental illness/severe emotional disturbance as well as co-occurring developmental disabilities. He/she will plan and provide psycho-educational groups in both the Day Treatment and Residential Treatment settings to facilitate the development of skills necessary to allow the individual child or adolescent to transition to – and function appropriately in – their home, school, and community. The Qualified Professional will provide other “Q” services within the Day Treatment and Residential programs as needed and will serve as a member of the multi-disciplinary treatment team.

Major Job Responsibilities: Develops and/or implements approved psycho-educational, educational/vocational, and treatment services which are of therapeutic value for the dually diagnosed population served by FACT Specialized Services and which are offered in accordance with state and federal standards. Develops and implements lesson plans to provide appropriate therapeutic services to the clients/students assigned to the Day Treatment Program. Determines, requisitions, and maintains adequate equipment, materials, and supplies needed for the provision of educational/vocational services in the Day Treatment setting in a cost effective manner. Serves as a member of the multi-disciplinary Treatment Team and attends the Team’s weekly meetings. Assists in the formulation and development of each client’s overall treatment plan, discharge criteria, and ultimate placement recommendations. Provides input for the Child and Family Treatment Team or attends such meetings upon request. Assists in establishing and maintaining productive community relationships and resources to support the program and the treatment goals and objectives of the clients. Provides psycho-educational groups in the Day Treatment and/or Residential Treatment settings and other needed support services as assigned and as appropriate in the role of QP. Documents treatment progress in the client record in a timely manner and in accordance with state and federal standards. Assesses, evaluates, and documents progress. Provides appropriate therapeutic and crisis interventions (based upon CPI principles/training) in the Day Treatment program setting. Documents such incidents as required by state and federal regulations and MHC Policies and Procedures. Participates in the on-call rotation to provide after-hours QP coverage (on-site assistance, crisis intervention, and post-restraint assessment) within the Residential program as the designee of the Clinical Director. Qualifications: Knowledge:
Bachelor’s degree in a Human Service field directly related to services for those with mental illness, developmental disabilities, mental retardation, special education requirements or similar populations
Experience:
• Prior experience in a treatment or educational setting serving children and /or adolescents, preferably providing direct clinical interventions or special education services to the specific populations served and;
• A combination of education and experience with the population served to meet the qualifications for a Qualified Professional as defined in 10A NCAC 27G .0104 of the North Carolina Administrative Code.

Maintain an as-needed availability status to client families (24-hour on call responsibilities with back-up available). Advocate for client’s needs within Methodist Home for Children service system, as well as other service systems. Coordinate service delivery with other agencies involved with the client, e.g., schools, mental health, juvenile court, DSS, etc. Participate in pre-service and in-service training for foster and therapeutic parents and staff. Adhere to all licensing standards and mental health standards. Provide detailed, routine, and quality documentation of planning and service activities to assigned foster and therapeutic foster homes and on behalf of clients residing in assigned foster homes, including oversight of the accuracy, timeliness, and quality of Medicaid treatment notes prepared by therapeutic foster families. Participate in regular review, update and monitoring of client service plan. Limited provision of “wrap-around” services (home and community-based services, one-on-one client services, based on client’s individualized service plan). Perform other duties as requested by supervisor.
